Oracle Forms and Reports are Fusion Middleware components needed for OFA installation. Forms is a GUI tool used to develop, generate and run database front end applications. Reports is a GUI tool used to develop, generate and run database reports. For the purposes of Oracle Fusion Applications installation we will secure Forms and Reports with Identity Management.
Once you download and unzip Oracle Forms and Reports software and run installer you will be presented with the welcome screen:

Forms installer will recognize WebLogic software we deployed in one of the previous steps and will create Forms subdirectory in Middleware directory:

We will create a new WebLogic domain for Forms and reports components:

You can pick and choose Forms and Reports components to be installed:

You will be asked to provide OID credentials:

Once post install scripts and patches are applied then installation progress screen will be presented - a number of operations are performed ( Forms and Reports Manged servers are created, Forms and Reports are deployed etc ):

When installation is completed you will be presented with a confirmation screen:
